The <\/span><a title=\"Caesarean guideline from NICE\" href=\"http:\/\/www.nice.org.uk\/guidance\/index.jsp?action=folder&amp;o=54526\" target=\"_blank\"><span style=\"text-decoration: underline; color: #0000ff;\">draft document<\/span><\/a><span style=\"color: #000000;\"> has been reviewed by\u00c2\u00a0stakeholders\u00c2\u00a0(of which I am one) and a\u00c2\u00a0new version is due out in Septemer 2011.<\/span><\/p>\n<p><span style=\"color: #000000;\">At first glance the draft is a marked improvement on the previous version, particularly now that it includes assessment of maternal request caesareans, this following active lobbying by members of the <span style=\"text-decoration: underline;\"><span style=\"color: #0000ff;\"><a title=\"Coalition for Childbirth Autonomy\" href=\"http:\/\/coalitionforchildbirthautonomy.org\/\" target=\"_blank\"><span style=\"color: #0000ff; text-decoration: underline;\">CCA<\/span><\/a><\/span><\/span> &#8211; of which I am a founding member. However there are still many gaps.<\/span><\/p>\n<p><span style=\"color: #000000;\">Of particular interest to\u00c2\u00a0me is\u00c2\u00a0how practitioners handle caesarean requests from women whose request does not fit in with any formally recognised medical reason. With regards tokophobia (fear of childbirth) improvements have been made &#8211; counselling is to continue to be available and now, where a woman continues to request a caesarean after this, her request should be granted. However, significant hurdles face those women who wish to discuss the possibility of a caesarean where the medical reasons are not clearcut or tokophobia cannot be proven. This is despite clear evidence that many requests originate from <a title=\"Women's Preference for a Cesarean Section\" href=\"http:\/\/onlinelibrary.wiley.com\/doi\/10.1046\/j.1523-536X.2001.00101.x\/abstract\" target=\"_blank\"><span style=\"color: #0000ff;\">previous trauma and pregnancy complications<\/span><\/a>.<\/span><\/p>\n<p><span style=\"color: #000000;\">The decision to request a caesarean is rarely taken lightly yet NICE currently continue to\u00c2\u00a0recommend that women undertake counselling. While balanced, comprehensive information and support during the decision-making process is of course crucial the guideline, as it currently stands in draft, implies that the role of\u00c2\u00a0this counselling is to change the minds of these women. It does not recognise that for some\u00c2\u00a0the most appropriate option is still a caesarean.\u00c2\u00a0These women\u00c2\u00a0will not be able to gain agreement for\u00c2\u00a0a caesarean\u00c2\u00a0till they near the end of their pregnancy, if at all.\u00c2\u00a0Resulting\u00c2\u00a0fear and\u00c2\u00a0distress may significantly impair their pregnancy experience, affect their emotional wellbeing and critically, in\u00c2\u00a0a small number of\u00c2\u00a0cases, may cause\u00c2\u00a0them to <a title=\"Fear of pregnancy and childbirth\" href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1742837\/?tool=pubmed\" target=\"_blank\"><span style=\"color: #0000ff;\">abort their baby<\/span><\/a>. <\/span><\/p>\n<p><span style=\"color: #000000;\">The UK health service needs to be clearer in its recognition that women have the right to make informed decisions about the way they give birth. They need to be more prescriptive about the information that is provided to ensure that it is truly balanced, discussing the risks of both caesarean birth <\/span><span style=\"text-decoration: underline;\">and<\/span><span style=\"color: #000000;\"> vaginal birth. Failure to do so\u00c2\u00a0leaves many women poorly prepared for their birth creating extra unnecessary\u00c2\u00a0trauma for those unprepared for a caesarean outcome (after all, 1 in 4 will experience a caesarean &#8211; many in emergency situations) .<\/span><\/p>\n<p><span style=\"color: #000000;\">The NICE draft is a step in the right direction but there are still some major issues with it.<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>NICE are updating their guidance on caesarean sections.
